About David Pilkington
Dr. David Pilkington, DO
Dr. David Pilkington is a board-certified Physical Medicine and Rehabilitation physician with fellowship training in Pain Medicine from the University of Washington. He specializes in diagnosing and treating acute and chronic spine, joint, nerve, and musculoskeletal pain using the latest evidence-based, minimally invasive techniques. His approach to care is centered on listening to each patient’s concerns, identifying the underlying cause of pain, and developing personalized treatment plans that restore function, improve mobility, and enhance quality of life rather than simply treating pain scores.
Dr. Pilkington earned his Doctor of Osteopathic Medicine degree with University Honors, graduating in the top 4–10% of his class at Western University of Health Sciences. He then completed his residency in Physical Medicine and Rehabilitation at the University of Washington, where he also completed an optional Medical Education Track and earned a Clinical Teaching Certificate, demonstrating his passion for education, leadership, and mentoring future physicians.
Following residency, Dr. Pilkington completed an advanced Pain Medicine Fellowship at the University of Washington, where he received extensive training in the diagnosis and treatment of complex pain conditions. He has expertise in a wide range of interventional procedures, including radiofrequency ablation, epidural steroid injections, sacroiliac joint injections, peripheral nerve blocks, ultrasound-guided joint injections, platelet-rich plasma (PRP) therapy, and botulinum toxin injections for migraine, cervical dystonia, and spasticity.
Throughout his career, Dr. Pilkington has remained committed to advancing the field of pain medicine. He has authored and co-authored peer-reviewed publications on chronic pain management, opioid stewardship, and rehabilitation medicine, presented research at national conferences, and delivered educational lectures on topics including fibromyalgia, pain pharmacology, cervical dystonia, and multidisciplinary pain management. He also co-founded the Puget Sound Spine Journal Club and has been actively involved in physician education and resident mentorship.
Dr. Pilkington is board-certified in both Pain Medicine and Physical Medicine & Rehabilitation and maintains active memberships in several leading professional organizations, including the American Academy of Pain Medicine, the International Pain & Spine Intervention Society, the American Society of Regional Anesthesia and Pain Medicine, the American Academy of Physical Medicine and Rehabilitation, and the American Osteopathic Association.
Outside of medicine, Dr. Pilkington enjoys hiking throughout the Sierra Nevada and Washington State, paddleboarding on alpine lakes, downhill skiing, and playing ultimate frisbee.
